Wormageddon is a Boss that can be found in Chapter 1
Behavior
Wormageddon stands still while doing the following attacks randomly:
- Summons up to 1/2/3
Round Worms if Wormageddon is below 100%/60%/30% max health, respectively. - Shoots a large blood shot towards Isaac, leaving a damaging blood creep on its way and explode into 8 blood shots in cardinal and diagonal directions.
- Burrows in the ground, leaving a small rock wave arround it, then follows Isaac for a while before digging out of the ground, leaving a large rock wave arround it.
- You can know when Wormageddon digs out of the ground when small rock waves starts appearing at its position.
Strategy
- In order to avoid getting overwhelmed by the Round Worms and the boss itself, you can use the large rock wave Wormageddon leaves when digging out to easily kill nearby Round Worms.

- Wormageddon is basically a boss version of the Worm family.
Delirium is able to transform into Wormageddon, and has a unique coloration when doing so.
